How would you describe the services you receive at ARE for Medical Case Management? (Check All That Apply)
How Easy Is It For You to Contact Your Case Manager and Schedule a Meeting For Services?
Do You Feel Comfortable Talking With Your Medical Case Manager About Things Like: Not Using Condoms All The Time, Cruising, Phone Apps You Use, Drug Use, Mental Health Needs, Etc.
Do You Understand the Disease Process of HIV?
ARE provides many ways for your voice to be heard in the services you receive. Some of those ways are: joining our ARE Board of Directors, joining our Client Advisory Committee (CAB), giving any grievances or suggestions to our agency suggestion box, speaking to your Case Manager or ARE staff member with suggestions, etc. Are you interested in learning about how you can get more involved?
How satisfied are you with the food selection in the pantry? Circle all that apply:
ARE will be starting a Positive Links app program in 2023. This phone app is available to all clients, is confidential, and helps track your medical information, as well as provides resources, frequently asked questions, allows you to privately message your Case Manager, and has a community post feature so you can connect to other clients of ARE. This app also has daily questions (did you take your meds, how is your stress level today, etc.?) that if you answer during the month you will be eligible for gift cards from ARE. Is this something you would be interested in joining?
Do you currently have any unmet needs that keep you from being adherent to your medical care and medications (such as going to the doctor every 6 months, taking your medication every day, etc.)?
ARE has business hours from 8:30am-5:00pm Monday through Friday and also offers virtual options for document signing/assessments, phone assessments, etc. for you to receive Case Management services. Do you feel you are able to access services either in-person or virtually so you are able to meet all your Case Management needs?
